GET THE THE STUDENT INTAKE FORM
I use the Student Intake Form to ascertain a few things: the logistics (how often will we meet, where will we meet, etc.); my student's interests—or what themes they want to study; and if my student wants bonuses like out of class work, copyediting, CV review, etc. I use the results of the Intake Form to create a package of lessons to suit my students wants and goals.
- Make a copy of the Student Intake form here.
- Save it to your Google Drive.
- To send to your own students, in the upper right corner of your quiz, click SHARE.
- In the pop-up box, click the link button. Click the Shorten URL box. Then click COPY.
- Paste the link into your New Student onboarding email.
GET THE ENGLISH PLACEMENT TEST
I use this English Placement Test to determine my students level, and even more importantly, the holes in their knowledge. I then use the results to create a course package to suit my student's grammar needs.

GET YOUR LESSON CONTRACT TEMPLATE
Once I get the results from the Student Intake Form and the English Placement Test to assess my student's wants, goals & needs, I create a 2-3 month "package" of lessons. The lessons should work towards a clear goal, so my student feels progress as we work together to achieve it. Then I fill out the Contract, create 3 package offer options and send a copy to my student. Unlike the forms, you must make a copy of each contract (just like any word .doc) and send the student their individual contract.

- Select FILE >> MAKE A COPY >> Name the copy with your student's name so you don't accidentally send one contract to a different student.
- Print or email it to your student.
- Keep a copy for your files.
